Transition Care and Hospice

photoTransition Care

HomePlace Special Care offers a place for peaceful recovery to individuals who are well enough to leave the hospital but not quite ready to care for themselves at home. We provide an environment with pleasing interiors that enhance the healing process. It's hard to know you are in a Alzheimer's care community except for the 24 hour on-site nursing. Activities that are helpful to the healing process will be offered to our guests.

Hospice

Hospice has been a part of HomePlace since the opening of the Burlington Alzheimer's community in August of 1997. At that time Skagit Hospice asked if HomePlace would be willing to develop a Hospice Care program. There was no hesitation on the part of the owners and soon HomePlace admitted its first Hospice guest. The staff feels a sense of fulfillment in their contribution to a necessary component of Skagit County.
