As necessary, Colonial Vista Rehab therapists consult with a patient’s physician to communicate progress and provide customized treatments.

The Colonial Vista campus includes retirement and assisted living apartments, a long-term care facility with a Medicare subacute rehab center, outpatient therapy center and a home-health agency. We are able to provide for our clients needs for services as those needs change in one convenient location.
